My collection form does not let anyone submit anything

Set up a form, sent it to two clients, both said there was nowhere to actually record or type. They got to the end and it just would not submit.

Had exactly this. Adding a question manually worked around it.

Confirmed and fixed — a form with no custom questions added never showed the recorder or the text box. Sorry, that one was bad. It now always shows a default prompt with both.
